My first visit
Please reserve approximately one hour for your new patient exam. We have made our new patient forms available to you as Adobe Acrobat/PDF files. For your convenience, we recommend printing and completing these forms at home. Please remember to bring your completed forms with you for your first appointment. If you have any problem getting these files to print properly, please call our office and we will gladly send or fax them to you.
Financing & insurance
All patients are asked to pay in full for their dental treatment. As a courtesy, we will gladly get a pre-determination of your insurance benefit for treatment and file a claim for your reimbursement. You may pay for your dental treatment by cash, check, Discover, MasterCard, Visa, Citi Health Card, LendingClub or CareCredit.
