Referral Process
- Download the Referral Form: To begin the referral process, please download our Referral Form. This form is designed to collect all necessary information to ensure a smooth transition for your patient to our care.
- Fill Out the Form: Please complete the referral form with as much detail as possible, including any specific treatment requests or patient needs.
- Submit the Form: Once completed, you can submit the form to us via email at office@sddis.com or fax it to (619) 598-0057. If you have any additional documents or patient records that need to accompany the referral, please include them with your submission.
- Confirmation & Scheduling: Upon receiving the referral form, our team will review the information and reach out to your patient directly to schedule their initial consultation. We will also send you a confirmation that we have received the referral and are in the process of contacting the patient.
